Q: What is the admissions process?


A: Our Admissions Department will coordinate all aspects of the admission process with you.  We’ll meet personally with you and your representative to help facilitate any required paperwork, complete your personal profile and assure a smooth transition to Boulevard Health Center. Please contact our Admissions Department at 248-852-7800 to start the process or answer questions that are specific to your situation.


Q: What should I plan to bring with me for my stay at Boulevard Health Center?


A: If you’re staying with us for rehabilitation, you’ll want to bring comfortable clothing that permits a free range of motion and movement such as loose fitting slacks, tops and sleepwear. This includes comfortable shoes such as athletic shoes. We also recommend you bring your preferred toiletries.


Q: What should I expect in the first few days of my stay?


A: Our team will begin with a personal assessment of your individual needs.  Together with you and your physician, we’ll develop an individualized plan that is based on meeting your goals and objectives.  Since a big part of getting well and going home is based on the successful completion of your therapy, we’ll get started on your therapy as soon as possible – usually in the first or second day of your stay.  The intensity and duration of your therapy plan is highly individualized, and you can rest assured that our team will work closely with you to accomplish your goals.  To learn more about our leading edge rehabilitation services, click here

Q: When can I expect to go home?


A: The length of stay is based on a number of factors including how quickly you regain your strength and mobility, what level of assistance is available to you when you return home, and the physical requirements of your home setting. We’ll work closely with you to assess your readiness to handle the tasks required in your home environment and help ensure your success.

Q: How do I pay for my stay?


A: Boulevard Health Center accepts Medicare, Medicaid, private insurance and private pay.  Please contact our Admissions Department  at 248-852-7800.
