When living independently is no longer an option, Boulevard Health Center offers comfortable rooms where your loved one can receive the compassionate care they need.  Our residents enjoy beautiful grounds, recently renovated rooms, lounges, and courtyards where they can welcome families and friends into their home.

Active Living

Whether it is a Wii bowling tournament, a trip to the cider mill, or a musical ensemble performing at Boulevard Health Center, our team, lead by our activities staff, provides a continual flow of vibrant and fun activities for our residents. To encourage socialization, we encourage residents to attend as many events as possible.
When a new resident arrives, an activity team member meets with them to understand their specific interests.  Based upon the resident interests and their health, the activity team member will invite them to attend specific scheduled activities, and work to create activities that the resident specifically will enjoy.   

Medical Care

Boulevard Health Center provides therapy 7 days a week and round the clock skilled nursing care.  Medical services include:
