Management Team

Boulevard Health Center is managed by Advantage Management Group (AMG), a Michigan based premier management firm that provides operation and finance management for short-term rehabilitation and skilled nursing centers.  AMG’s passion is to combine innovative ideas with compassionate medical care and state of the art programs to create a place of healing and hope for those who need rehabilitation and nursing care.  AMG’s staff works closely with community hospitals and physicians to create a collaborative environment of care and medical expertise.

Our Leadership Team

Medical Director: Dr. Fadi Salloum, M.D.

As Boulevard Health Center’s Medical Director, Dr. Fadi Salloum stays current with the latest trends in geriatric care and patients benefit from his 17 years of clinical experience. He received his medical degree from the Damascus University School of Medicine in Damascus, Syria in 1993. Dr. Salloum completed his residency program at Grace Hospital/ Wayne State University in 1998.

Dr. Salloum is board certified in internal medicine by the American Board of Internal Medicine. He is also a professional member of the American Medical Association and the American College of Physicians. He has been practicing as a hospitalist since 1998, allowing him vast patient care knowledge and experience. He specializes in the care of hospitalized, nursing home, assisted living and hospice patients. He also provides care to patients living in senior living communities. In addition to being Boulevard Health Center’s Medical Director, Dr. Salloum has hospital privileges at POH Regional Medical Center, St. Joseph Mercy Oakland, Crittenton Hospital and Select Specialty Hospital. His well-rounded professional training and vast experience help him assess each patient’s needs and assist in the creation of an appropriate medical plan. [more]

Director of Rehabilitation: Dr. Craig Hysni, M.D

As Boulevard Health Center’s Director of Rehabilitation, Dr. Craig Hysni oversees Boulevard’s extensive rehabilitation services including new methodology and protocol development. As a board certified Physical Medicine and Rehabilitation physician, he focuses on the nonsurgical diagnosis and treatment of musculoskeletal and spinal disorders. He also works with patients after surgery to maximize the effectiveness of their therapy treatment.

Dr. Hysni is the Associate Medical Director of inpatient rehabilitation at Crittenton Hospital. He is active in teaching medical students and residents. He has been named by Hour Magazine as a “Top Doc”.

Rehabilitation Manager:  Kathy Seeber, OTR, MBA

As Boulevard’s Rehabilitation Manager, Kathy Seeber personally brings more than 19 years of clinical experience in the field of Occupational Therapy. Ms. Seeber and her team are well versed in the rehabilitation of adult patients with orthopedic, neurological, cardiac, and traumatic injuries along with those patients with medically complex diagnoses and debilitation. Ms. Seeber oversees the daily management of a rehab team that collectively offers over 120 years of experience in Physical Therapy, Occupational Therapy and Speech Therapy.

Nursing Director:  Carolyn Jones

In her role as Boulevard Health Center’s Director of Nursing, Carolyn Jones ensures that each patient’s individualized care plan is implemented. She oversees clinical assessments, intervention and the outcome of all nursing care. Ms. Jones is a Registered Nurse and has been in long-term care for 25 years, and worked as Nursing Director since 2003. Under her direction, the Boulevard Health Center’s nursing staff takes a pro-active approach to providing first-class care and comfort for all patients.

Boulevard Cardiopulmonary Clinic:  Dr. Samer Kazziha, M.D., FACP, FACC, FSCAI, FCCP, FSVMB Cardiologist

Sam Kazziha, MD takes great satisfaction in being able to provide the latest in treatment technology to those with cardiovascular disease.
Dr. Kazziha is the Executive Medical Director of the Cardiovascular Program at Crittenton Hospital Medical Center’s Shelton Heart Center. He is regionally recognized for his excellence in both clinical and administrative roles, and he has developed cardiovascular programs at several hospitals.

Dr. Kazziha is a diplomat of the American Board of Internal Medicine, the American Board of Cardiovascular Disease, the American Board of Interventional Cardiology and the American Board of Endovascular Medicine. He is a Fellow of several professional organizations, including the American College of Cardiology, the American Society of Coronary Angiography and Intervention, the American College of Chest Physicians, the American College of Physicians, and the Society for Vascular Medicine, and is a Member of many others.

Dr. Kazziha is a frequent symposium speaker for healthcare professionals and also conducts AHA endorsed “Heart of Health” monthly lectures for the public. He is active in many aspects of cardiovascular clinical research and his work is widely published.

Pulmonologist, Boulevard Cardiopulmonary Clinic:
Dr. Robert Go, M.D., FACP
